Right Femoral Alignment Osteotomy & 
Repair of Right Superficial Flexor Tendon and Achilles

Case Information:  

Female Golden Retriever, one year old, weight 81 pounds.  Presented for hindlimb lameness.

Evaluation:

Owner reported limping after going for walks, and that the dog would fall down when it "turned fast", three months duration.  Physical examination showed a bowlegged conformation, positive sit test bilaterally and bilateral Grade II medial patellar luxations.  Radiographs revealed bilateral double condyle signs and bilateral distal femoral varus.

Re-evaluation of the right hindlimb prior to surgery revealed a superficial flexor tendon luxation and early achilles tendon tear in addition to the patellar luxation and varus deviation of the right distal femur.

Diagnosis:  

Right Grade II medial patellar luxation secondary to distal femoral varus, right superficial flexor tendon luxation and early right achilles tendon tear.

Treatment: 

Right 20° distal femoral medial opening wedge osteotomy.  Locking loop suture both medial and lateral from calcaneus to superficial flexor tendon; malleolar screw for extension of hock during healing, screw removal at 7 weeks.

Recheck Examination:

Five month follow-up.  Owner reported that dog had no pain or stiffness after a full day of activity.  Physical examination showed that dog was able to jump into the car and take stairs with no difficulty.  Dog was walking with normal weight bearing and no abnormalities noted.
