TPLO plus Distal Femoral Alignment

Case Information:  

Female Pit Bull, one year old, 59 lbs.  Presented for hip examination.

Evaluation: 

Owner reported that dog was experiencing morning pain and diminished activity.  Dog was walking with normal weight bearing on both hindlimbs at examination and experienced no difficulty going up stairs, but was unable to jump into the car.  Abduction external rotation test normal - right and left hips.  Sit test positive (see stifle exam), stand test okay.  No angle of reduction or subluxation - right and left hips.  Radiographs revealed no acetabular filling - right and left.  DAR angle right hip 4°, DAR angle left hip 7°.  Lateral radiograph - white/black/white joint space on both hips.  No femoral osteoarthritis - right and left, joint congruency 4/4 - right and left.

Examination of the right and left stifles revealed positive sit test on the right stifle.  Cranial drawer sign and tibial compression test positive on right stifle.  Palpation of the medial meniscus positive on right stifle.  There was atrophy of the right biceps femoris musculature, and medial patellar luxation.  Radiographs of the right stifle revealed lateral tibial plateau slope of 30°, an inflamed fat pad, osteophytes and an inflamed caudal meniscus.

Diagnosis:

Partial rupture of the right cranial cruciate ligament and right Grade III medial patellar luxation secondary to a distal femoral varus and excessive tibial plateau slope.  Normal hips.

Treatment:  

Right Tibial Plateau Leveling Osteotomy plus distal femoral valgus osteotomy.  No arthrotomy performed, 8° of varus corrected at TPLO site.  Routine distal femoral valgus osteotomy using TPLO jig.

One year, ten month follow-up, mature Tibial Plateau Leveling Osteotomy and distal femoral alignment osteotomy.
