Sept 12-13th TPLO course now accepting registration. | External Fixator for Lengthening of Jaw to Match Bite Click on images to enlarge for viewing. Case Information: Five month old, male Siberian Husky. Presented for underbite. Evaluation: Physical examination revealed a short mandible by 15mm. The canine teeth were contacting the roof of the mouth. Diagnosis: Short mandible which was preventing proper occlusion of the canine teeth. The caudal and medial malpositioning was causing an erosion through the soft palette. Treatment: A Slocum® external fixator was placed on either side of the jaw. Two intrameduallary pins were placed caudal to the fourth premolar, and three were placed cranially to the third premolar. A threaded bar was used for lengthening. The symphysis of the mandible was opened with an osteotomy. An osteotomy of the mandible between the third and fourth premolar was performed on both the right and left sides in order to distract the mandible from that location. Ten days following initial surgery, a gingival flap technique was performed to enable the gingiva to keep up with the distraction. After five days to allow for soft tissue healing, the distraction was continued. Bone grafts were added to the mandibular osteotomies after desired length was achieved at one month following initial surgery. Four month follow up: External fixator removed. The mandible was healed and there was excellent matching of the bite on both the right and left side.
